Dihydropyridine is a "slow" calcium channel blocker with antihypertensive and antianginal effects. It blocks calcium channels, reducing the passage of calcium ions through the membrane into the cell (to a greater extent in vascular smooth muscle than in cardiomyocytes).
The antihypertensive effect is directly related to the vasodilating effect on the blood vessels. Has a long-term dose-dependent antihypertensive effect.
Amlodipine reduces myocardial ischemia (has an antianginal effect) in 2 ways:
1) dilates peripheral arterioles, thereby reducing the total peripheral vascular resistance, reducing the subsequent load on the heart, with almost no change in heart rate, which leads to a decrease in energy expenditure and myocardial oxygen demand.
2) dilates coronary and peripheral arteries and arterioles in unaltered and ischemic areas of the myocardium, which increases oxygen supply to the myocardium (especially in vasospastic angina), reduces the severity of myocardial ischemia, and prevents coronary artery spasm (including smoking).
In arterial hypertension, a single dose provides a clinically significant decrease in blood pressure (AB) within 24 hours (in the "lying" and "standing" position of the patient). The slow onset of action of amlodipine does not cause a sudden drop in blood pressure. It does not reduce tolerance to exercise and the volume of blood flowing out of the left ventricle.
In patients with angina, a single daily dose increases the duration of exercise, prevents the development of angina and ST-segment "ischemic" depression, reduces the frequency of angina attacks and nitroglycerin consumption.
Reduces the level of left ventricular hypertrophy.
