Intrauterine spiral - what it is, the advantages and disadvantages of the spiral, contraindications

  • Mechanism of aseptic inflammation. Once the BIS is installed, leukocytes begin to enter the uterine cavity, looking at the spiral as a foreign body. Leukocyte infiltration of the endometrium prevents implantation of the egg. In addition, leukocytes, macrophages and histiocytes enhance sphagocytosis of sperm, thereby increasing the contraceptive effect of BIS.
  • Enzymatic disorders. BIS alters the amount of enzymes in the endometrium and creates conditions that negatively affect sperm, egg cell survival.
  • Movement of prostaglandins. Once BIS is established, biologically active substances - prostaglandins - begin to be synthesized intensively, a substance that affects many stages of reproduction.
  • Inhibition of ovulation. The established BiS affects the hypothalamic-pituitary system and complicates the production of gonadotropins, correspondingly to the formation of sex hormones, resulting in pre-ovulation.
  • Inhibition of sperm migration into the cervical canal. The ends of the spiral strands make it difficult for the sperm to enter the uterine canal. When the Mirena hormonal coil is inserted (levonorgestrel stores), the cervical mucus thickens in the uterine canal.

Spiral placement conditions and inspections

Before the introduction of BIS, the woman must undergo the necessary examination:

  • Collect anamnesis to identify possible contraindications;
  • Gynecological examination to determine the location and size of the uterus;
  • Bacterioscopic examination of discharge from the vagina, cervix or urethra;
  • Clinical urine and blood tests;
  • Blood tests for HIV infection, syphilis, and hepatitis B;

  • Requires one permanent sexual partner;
  • Spiral insertion is not recommended for infertile women;
  • Increases the risk of ectopic pregnancy;
  • Increases the risk of inflammatory diseases of the lower pelvic organs;
  • Requires monthly independent monitoring;
  • Reduces endometrial fullness, which reduces the chances of pregnancy.
