First, since you only have 30 minutes to fill out the questionnaire, please clarify the following.

Tips to keep in mind when filling out questionnaire questions:
1. Last/Family Name – surname; First Name - name. Middle Name is a second name, it does not apply to Uzbeks, therefore it should be specified as No Middle Name.
2. Indicate your gender: Male – male; Female - a woman.
3. Date of birth (entered in numbers): Month, Day, Year. Note that the month is written first.
4. City of your birth. The name of the city or settlement must be written in the Latin alphabet, words such as "district", "shahri" should be included.
If your birth city is unknown, check the Birth City Unknown box.
5. Country of your birth. Select Uzbekistan from the list (Uzbekistan).
6. Paragraph six is ​​for persons whose country of birth is not on the list of countries participating in the lottery. Citizens of Uzbekistan will be checked automatically.
7. Passport information. Enter your first and last name as in step 1 and check the No Middle Name box.
Passport Number – series and number of your biometric passport for going abroad;
Passport Expiration Date - passport validity period (month, day, year);
Country/Authority of Issuance – passport issued by Uzbekistan.
Stateless persons should select the first option (Stateless) by ticking the box in Section 7, Part B.
8. Photography. Post a photo of yourself taken within the last 6 months. (photo should be in JPEG format, maximum size 240 KB, size from 600×600 to 1200×1200 pixels, 24 bit color depth);

9. Mailing address.
In Care Of - the name and surname of the person at the postal address is written here. You can also write your name and surname on it.
Address Line 1 – this item shows the street name, house and apartment number. If the place or street name does not fit on the first line, you can write it on Address Line 2.
City/Town – the name of the place of residence.
District/Country/Province/State – the name of district/country/region/county (state).
Postal Code/Zip Code – postal code. If you don't have a zip code for where you live, check the No Postal Code/Zip Code box.

10. Country Where You Live Today - The country where you live today.
11. Phone Number - you must enter your phone number in international format (+998901234567).
12. E-mail. Copy and write your email address in the first line, and again in the second line.
13. What is your highest level of education to date? It is necessary to choose one of the indicated options.
• Primary school only
• High School, no degree – high school, but no certificate
• High School, degree – high school, has a certificate
• Vocational School – professional college
• Some University Courses – part-time higher education (undergraduate students choose this item)
• University Degree – higher education (for citizens with a bachelor's degree)
• Some Graduate Level Courses – some graduate level courses (master's degree students choose this item)
• Master's Degree – master's degree
• Some Doctorate Level Courses – Incomplete postgraduate studies/doctoral studies (postgraduate and doctoral researchers choose this item)
• Doctorate Degree – doctorate degree
14. Your marital status. It is necessary to choose one of the indicated options.
Unmarried - I have never been married.
Married and my spouse is NOT a US citizen or US Lawful Permanent Resident (LPR).
Married and my spouse IS a US citizen or US Lawful Permanent Resident (LPR).
Divorced - I am divorced.
Widowed – My spouse died.
Legally Separated – There is no legal basis for the status of Legally Separated in the CIS countries, therefore, specifying this item may cause the application to be rejected during the interview process.
15. Number of Children - the number of children. Here you list the number of all your children under the age of 21. In this paragraph, write the number of your children in numbers, and if there are none, enter zero.
We remind you that if your spouse and children are indicated in the paragraphs, the second part of the application will be opened. In the second part, their information must be filled in as above
After all the processes are completed, the Continue button is clicked. You will then be given the opportunity to review your application again. Once you're sure everything is in place, you can click the green Submit button.
As there is a result verification code on the next page, we advise you to take a picture or print it immediately.
Lottery results will be announced on or about May 2023, 8 at dvprogram.state.gov.
